Impact: Some FYI users received a system notification for an error uploading documents.
Current Status: This issue has been resolved.
Root Cause: The issue was caused by a back-end instance processing emails without the required access, which caused the FYI audit service to detect incorrectly uploaded emails and triggered system notifications.
Due to the asynchronous nature of the FYI email service, the majority of these emails were subsequently uploaded.
Planned Actions: Emails that did not upload originally have been re-processed successfully, and our engineers are working with our providers to determine what caused the instance to run without the required access.
The cause of this issue has been identified and we have prevented any new emails from reporting the error.
Workaround: The majority of emails that received the error message were uploaded correctly in FYI, in which case the error can be ignored.
A small number of emails have not been uploaded into FYI correctly, our engineering team are reprocessing these emails so they are uploaded into FYI - This is expected to take approximately 2 hours.
We will notify any affected users once this has been completed.
